1. If you have a problem or complaint, or if you need maintenance, please download, fill out and print this Complaint/Service Request form (CSR) or call management at the number listed below to report it.

2. If, within 30 days, your issue goes unresolved, no action is taken, or you are dissatisfied , please take the following steps.

   • Ask to be put on the agenda for the next monthly board meeting so time can be allotted for your concerns.
